Following your annual Branch Meeting, please complete the following forms (see links below). You will need to “open” the file in Adobe Reader or Adobe Acrobat, print the form, complete and either mail, fax and e-mail the forms. Please complete the forms in their entirety.

1. Agenda - No District Meeting (Dist 2,3,4,7,8 and 9)

3. Delegate Credential Form Pg 1 (delegates 1-3) Districts 1, 5 and 6 will have a district meeting in 2018 and will hold director elections. The branch packet you received by mail includes the number of delegates (based on membership) your branch may elect.
   
   For branches electing more than 3 delegates, additional forms must be completed.

4. Alternate Credential Form See number 3 above. An alternate will replace any delegate that might later be unable to attend the District Meeting.
